He was predeceased by: his parents, Margaret Kozlovsky (Kennedy) and William A. Kozlovsky; and his brothers, James F. Kozlovsky and Thomas A Kozlovsky (Yvonne). He is survived by: his children, Susan Kozlovsky Husk (Mike Husk), Bill Kozlovsky (Ann Ducharme Kozlovsky), Patricia Kozlovsky (Paul Martin), Janette Blee (Robert Blee) and Kaela Kozlovsky Bernal (Mark Bernal); his grandchildren, Kirsten Kozlovsky Abercrombie, Karen Kozlovsky, Christina Kozlovsky, John Blee, Shannon Blee, Mary Blee, Aidan Bernal and Declan Bernal; his great grandchild Edward Abercrombie; and his wife Teri.
