He is survived by : his father Rodney Blake (Annette); his wife Andrea Blake; his stepchild Kelsea Ingle; his mother Patty Lefever (Ken); his brother Chase Blake; his stepsiblings, Brian Sampson, Braden Sampson and Brandi Wagner; his mother-in-law Donna Wells; his sisters-in-law, Ashley Young and Amber Fife; and his grandparents, Jack (Gramps) and Twyla Blake. He is also survived by many aunts, uncles, nieces, nephews, cousins, "brothers", friends, and co-workers.
