He was predeceased by: his parents, Nathan G. Cook and Lula Mae Cook (Reaves); and his siblings, Ruby Jean Smith, Juanita White, Nathan Cook, Donald Cook, Freddie Cook and Herman Cook.

He is survived by: his wife Norma Cook; and his brother Ricky Cook of Eastman, Georgia..
