He is survived by: his parents, Rory Schmalzried and Dawn Schmalzried of Castle Rock, Colorado; his siblings, Heath Schmalzried (Amanda) of Concord, Tyler Schmalzried (Amber) of Kiowa, Colorado and Hope Payne (Norris) of Elizabeth, Colorado; and his grandmother Rozan Schmalzried of Quinter, Kansas. He is also survived by numerous nieces, nephews, aunts, uncles, and cousins.
In lieu of flowers, memorial contributions are suggested to Sandstone Care Denver, an addiction treatment center that helped Jack, or to Gove County Medical Center.
