He is survived by: his children, Kyle and Trevor; his parents, Julie Kubaska and David Kubaska of New Milford, CT; his brothers, John (Mary) and Kenneth; his former wife Lisa Kubaska; his nieces and nephews, Alex, Ellie, Lillian and Andrew Kubaska; and his uncles, Stephen (JoAnn) and Michael Kubaska (Nora).
