He is survived by: his father William Cook; his significant other Paula Cook; his son Mathew; his daughter-in-law Chanda Cook of Wausau, Wisconsin.; his grandchildren, Jayden, Preston, Dillon and Michelle; his great grandchildren, Bentley, Brooklyn, Malaya and Hunter; his siblings, Patty Michaud (Greg) of LaPorte and Gary (Brigette) of Durango; his sister-in-law Elaine Cook of Colorado Springs, Colorado.; Patty (Excitement). He is also survived by his family, friends.
