He was predeceased by: his uncle John R. Rook; and his pet Latte. He is survived by: his parents, Maggie Rook and Thomas Costa; his daughters, Skylar and Ryalyn Rook; his sisters, Brittany and Julia Flynn; his half-sister Courtney Carvahlo; his aunt Kathleen (Rook); his uncle Kevin Mullany; and his cousins, Courtney, Maura, Ashley Rook, Sean and Erin Mullany. He is also survived by two nieces, one nephew, and his second cousins.
