He was predeceased by : his parents, Roman Kotelko and Marie Kotelko. He is survived by : his wife Kathi; his former life partner Karen; his children, Dr. Alexis Kotelko, Nick Kotelko (Angie), Dr. Cody Kotelko and Kaitlyn Kotelko; his brothers, Zenon Kotelko (Michelle) and Terry Kotelko; and his pets, Milo and Gracie.
