He was predeceased by: his parents, James Cook and Jeannine Cook; and his siblings, Daniel Cook (Patti) and Ann Ramirez (Ricco). He is survived by: his wife Clarice DalPra Cook; his sons, Shawn (Maria Rosell) and Ryan (Kristi Ryder); his grandchildren, Luna, Maja, Ryder and Saylor; and his siblings, Colleen Stinson (Tom), Theresa Cook Dalton and John Cook (Karla).

In lieu of flowers, donations to the family will be used for medical expenses, and to support cancer and kidney research in Michael's honor.
