He is survived by: his wife AJ; his children, Alissa, Rachel, Derek, Jeremiah Smith (Karen), Bart, Jeremy Smith, Jerusha and Angelica Smith-Hendricks (Riley); his sibling's children, Meghan, Terrill, Aaron, Josh and Jace; his goddaughters, Tasha Roettger (Michael), AmeliaWeltch, Yvonne Armour and Hannah Zepp of Germany; and his granddaughter Mia Bella Polizzi.
A memorial service will be held on Friday, November 14th 2025 at 10:00 AM at the St. Mary Catholic Church (1855 St Marys Dr, Montrose, CO 81401).
