He was predeceased by: his parents, Joseph L. Sullivan and Verna L. Sullivan (Holtmann); his grandchildren, Hailey Rene, Kyle Lee, Patrick Riley, Blake Lee, Wyatt James, Paisley Rain and Payton Rylie; and his great grandchild Amaria Rae.
He is survived by: his wife Theresa Lynn Warburton; his children, Shawn Patrick (Cheri), Jeffery Dennis (Crystal) and Rebecca Lynn (Robert); and his mother-in-law Pat Warburton.
