He is survived by: his mother Ruth Perez of San Luis, CO; his wife Jessica Perez; his daughters, Nina, Mia and Sophia Jo of Blanca, CO; his brother Jerry Perez (Sandy); and his sibling's children, Josiah Medina, Ben Perez, Jon Perez, Sade Perez and Kierstin Medina. He is also survived by numerous cousins and friends.
