He was predeceased by: his parents, Abe Levine and Sally Levine; his wife Glenda Levine; and his brothers, Marvin and Norman.
He is survived by: his children, Deborah Doty (Jeff Doty) and David Levine (Laura Rowlen); his granddaughters, Chelsey Doty, Amy Knight (Terrell Knight) and Elise Levine; and his great grandchildren, Ava, Abigail, Benjamin and Annabelle Knight.
In lieu of flowers, the family asks that a donation to the Ronald McDonald House Charities of Nashville be considered.
