She was predeceased by: her husband Edward J. Bernardo; and her parents, George Jack and Mae Diehl Jack. She is survived by: her children, Edward G. Bernardo (Jessica) of Southington and Kim Mitchell (Kenneth) of Ansonia; her grandchildren, Jason Provenzano of Ansonia, Nicole Dery (Ryan) of Simsbury and Amanda Lynn Bernardo of Prospect; and her great grandchildren, Gavin, Jameson and Anthony Dery.
