She was predeceased by: her husband Michael J. "Birdie" Ptak; her parents, Walter Milek and Mary Brejnak Milek; her siblings, Stanley, Alexander, Benjamin, John Milek, Virginia Siekierski, Jennie Cordner, Agnes Saddig and Sophie Sobireski; and her siblings' children, Jeffrey Saddig, Leon "Junior" Siekierski, David Siekierski and Cindy Sobireski.
She is survived by: her son Mark A. Ptak (Terri) of Seymour. She is also survived by nieces and nephews.
