He was predeceased by: his parents, Pasquale Forte and Constance DeMauro (D'Addario); and his siblings, Mary Peters, Anne Freedman, Lucy White, Albert and George D'Addario.
He is survived by: his nephew George D'Addario Jr. (Maria); and his caregivers, Meghan and Brianna. He is also survived by several nieces and nephews.
