He was predeceased by: his parents, Anna Cook and Samuel Cook; his mother-in-law Barbara Kahoun; his brother-in-law Michael Jones; and his nephew Adam Jones.
He is survived by: his wife Karen; his sons, Andrew and Jacob; his siblings, Cathy Usinas, Vera Jones, Sam, Alan and Hugh Cook; and his siblings-in-law, Scott, Greg Kahoun, Kathie, Sarah Kahoun and Theresa Klan.
