He was predeceased by: his parents, Rafael Perez and Cristina Diaz. He is survived by: his daughter Rosa Mayra Collins; his children-in-law, Michael Collins (Julio Patron), Ana Patron (Harry Rivas) and Maribel Rivas; and his siblings, Rhona Rodriguez, Doris Medina, Gladys Marquez, Rene Medina and Vincent Medina. He is also survived by 8 grandchildren; 11 great grandchildren; and a host of extended family and friends.
