He was predeceased by: his father Dr. William Stevens. He is survived by: his mother Sallie Stevens; his wife Dr. Leila Garrett-Stevens; his sons, William and Owen; his sisters, Kristin Stevens, Stephanie Stevens and Heather Clayton (Carter); and his siblings' children, Erin Tippett (Brian), Emily, Ella Clayton, Sarah (Adrian), Charlotte Robinson, Mallory O'Shea and Tristan O'Shea.

In lieu of flowers, the family requests donations be made to Camp Dudley, Westport, NY, https://www.campdudley.org in Mike's name.
