He is survived by: his wife Anne Mikullitz (Cusack); his children, John Mikullitz (Patricia) of Boston, MA, Mark Mikullitz (Carrie Durant) of Larchmont, NY and Kathleen Durkin (Edward) of Montclair, NJ; and his grandchildren, Cadence, Madeline, Frances, William, Alexander and David.
