He is survived by: his parents, Ana Delia Martinez and Luis Velez; his girlfriend Santa Arroyo; his children, Evelyn Melendez (Jose) of Leominster, MA, Miguel Velez, Jr. of Ocala, FL and Jose Antonio Velez (Karina Guerrero Fernandez) of Puyallup, WA; his grandchildren, Jose Melendez, Jr. (Tiana Tyndall), Josalyn Melendez, Laila Melendez, Dr. Miguel Velez, III (Jacqueline Rose Velez), Brittany Lee Jace (Meagan Jace), Yaneily K. Velez (Yonan Guzman Padilla) and Antonio Gene Velez; his great grandchildren, Bella Bonilla, Elianna Velez, Yonan Fanuel Guzman, Jr., Kaya Melendez and Miguel Velez, IV; and his sisters, Ramonita Velez, Ana Maria Velez-Collazo, Carmen Maria Velez (Edwin Barreto), Marisol Velez and Yvette Martinez. He is also survived by his grandchildren and great-grandchildren.
