He was predeceased by: his parents, Ferdinand Slezak and Saima Slezak (Heikkila). He is survived by: his close friend Donna Raymond; and his brothers, Steven Slezak (Gail), John Slezak (Carolyn), Robert Slezak (Karen Sweet), Richard Slezak (Kerri Lynn) and Walter Slezak. He is also survived by many nieces and nephews.
