He is survived by: his wife Chiao-Ling Blazejowski; his sons, David Alvin Blazejowski (Heather Burch) and Jason Mikel Blazejowski, Sr. of Meriden, CT; his grandson Jason Mikel Blazejowski, Jr. (David); and his brother Mark Blazejowski (Jennifer) of Herndon, Virginia.. He is also survived by several nieces and nephews.
