He was predeceased by: his parents, Arthur A. Cook and Frances Cook (Gulesian); and his former wife Shirley Cook (Fox). He is survived by: his wife Eleanor Cook (VanMol); his sister Betsy Ann Cook of Philadelphia, PA; and his stepchildren, Annie DuBois (Robert Tedesco) and Tim VanMol (Ann). He is also survived by step-grandchildren and great-grandchildren, nieces, nephews, and dear friends.
