He was predeceased by: his parents, Joseph Formato, and Florence Formato, Sr. (Cavalier); and his sister Dorothy Ann Parente. He is survived by: his wife Elaine Formato (Malinowski); his children, Denise Valerie Botti, Robert Joseph Formato and Donna Lynn Formato; and his sisters, Connie Civitello and Anne Amore. He is also survived by eight grandchildren, and sixteen great-grandchildren.
