Sivak; his mother-in-law Angeline Caruso; his siblings, Daniel Sivak, Mary Ann Salerno and George Sivak (Donna); his siblings-in-law, Diane DeLuca (Mario Infanti) and Michael Caruso; his nieces and nephews, Anthony, Daniel Caruso, Michael Sivak, David Salerno, Amy Anderson and Tracie Hoey; his close friends, Roy, Kathy Lenhard and John; and his caregivers, John, RNs, Maria, Helen, Brian, Kristina, Aides, Gladys, Diana, Elsa, Elizabeth, Carlos and Nadine.
