He was predeceased by: his father Edward. He is survived by: his mother Patricia Jakubczyk Sr. (Boucher); his siblings, Donna Pelletier of Oakville, Lorie Rafael (Armando Rafael) of Naugatuck, Michele Jakubczyk of Greenville, SC and John Jakubczyk of Waterbury; and his cousin Gary Oddon of Stratford. He is also survived by many nieces and nephews.
