He was predeceased by: his parents, Dr. Louis Forman and Beatrice Forman. He is survived by: his wife Bayle Mann; his daughter Hilary Schaffer (Andrew); his granddaughters, Emma and Chloe Schaffer; his sister Eileen Klein (Stuart); his brother-in-law Arnold Mann (Jeanne); and his nibling Callie.
A graveside service will be held on Friday, October 2nd 2026 from 10:30 AM to 11:30 AM at the B'Nai Jacob Memorial Park (200 Wintergreen Ave, New Haven, CT 06511).
In lieu of flowers and gifts, Jerry asked that donations be made in his name to: St. Jude's Children's Hospital, ASPCA and Wounded Warriors.
