We bid farewell to Hazel M. Johnson (Putnam, Connecticut), whose journey of life gracefully concluded on July 1, 2025 at the age of 96. Hazel touched countless lives with warmth and kindness, leaving behind cherished memories that will forever echo in hearts. She was predeceased by: her parents, Elsie Merrell (Burger) and Colin Merrell; her husband Mike Blake; her former husband Carl Oden Johnson; and her siblings, Jean Buckley (Tim) and George Merrell (Jeanette).
She is survived by: her sons, Mark and Dana Blake (Patti); her grandchildren, Madison (Logan) and Laura Blake; her great grandchild Avi Blake Vanderlaan; and her caregivers, Joy, Faye and Shakira.
A celebration of life will be held on Saturday, August 23rd 2025 at 10:30 AM.
