He was predeceased by: his parents, John A. III and June Kostick, III (Pijar). He is survived by: his wife Cynthia Kostick (Morin); his children, John A. Kostick V and Marya Kostick of Seymour; his brother Jeremy Kostick (Holly) of Avon, NC; and his nieces, Paisley Kostick of Archdale, NC, Airman and Class Chloe Kostick of Columbus, AFB.
