He was predeceased by: his parents, Marie Sladek and William Sladek. He is survived by: his wife Marilyn Sladek; his children, Kevin Sladek (Tina), Keith Sladek and Pamela Howe (John); and his grandchildren, Elizabeth Sladek, Ian Howe, Kailyn Howe, Zackary Sladek and Madison Sladek.
