He was predeceased by: his wife Karen Pjatak (Ricitelli); and his parents, John Pjatak and Marcella Pjatak (Gallanger). He is survived by: his children, Richard D. Pjatak (Crissy), Kevin G. Pjatak (Jen) and Tammy A. Pjatak; and his brothers, John and Tom Pjatak. He is also survived by nieces and nephews.
