He is survived by: his wife Joyce; his sibling Gildo Consolini; his children, Eileen Meade (Jeffrey), Susan Szabo (Richard), Lisa Champigny (David), Melanie Felladore (Scott), Amy Cardello (Michael) and Scott Consolini (Leslie); his son-in-law Paul Beechinor; his son Michael Yuchunis; his grandchildren, Michael, Paul Jeffrey (PJ), Jessica, Samantha, James, Anthony and Sydney (Skidder); and his great grandchildren, Lucas and Eli.
In lieu of flowers, the family requests donations be made to Jack and Joyce's favorite charity, St. Jude's Children's Hospital.
