He was predeceased by: his wife Irene Gaszek Koczak; and his parents, Arthur Koczak and Lorraine DesJardins Koczak. He is survived by: his children, Richard Koczak (Andrea Falcon) of Wallingford and Tatiana Koczak (Jim Potter) of Canton; his grandchildren, Nina, Samantha, Nicole Koczak, Madison, Alyssa, Julia and Tyler Koczak; and his siblings, Arthur Koczak (Lorraine), David Koczak (Mary Jane), Cheryl, Cindy, Dennis and Mark Koczak.
Visitation will be held on Sunday, October 19th 2025 from 2:00 PM to 5:00 PM at the Wallingford Funeral Home (809 N Main Street Ext, Wallingford, CT 06492).
