He was predeceased by: his parents, Eleanor Dembek and Frank Dembek. He is survived by: his significant other Ann Nolan; his brother Robert Dembek (Lucy); his nieces, Allison Dembek and Melissa Guevara (Juan Pablo); his great-nephew Matias Guevara; and his extended family, Kevin, Edith Nolan, Kimberly, Louis Nassetta, Kerri and David Bond. He is also survived by eight grandchildren, and seven great-grandchildren.
In lieu of flowers, donations in Tom's memory may be made to the Goshen Fire Department Inc. Scholarship Fund.
