It is always difficult saying goodbye to someone we love and cherish. Family and friends must say goodbye to their beloved Thomas J. Kozaczka of West Suffield, Connecticut, who passed away at the age of 76, on June 4, 2025. You can send your sympathy in the guestbook provided and share it with the family.

He was predeceased by: his parents, Joseph Kozaczka and Nellie Kozaczka; his sister Jo-Ann Chiaravallo; and his nephew Brian Kozaczka. He is survived by: his brother David Kozaczka (Sharon) of Suffield; and his sibling's children, Jeremy, Steven, Matthew, Zachary Kozaczka, Dyan Kozaczka and Amy Chiaravallo-Glayat.
