He was predeceased by : his father William R. Jr. Chalk. He is survived by : his mother Ivy L. Chalk; his siblings, Carol Chalk-Bates, Michael B. Chalk and Cheryl L. Shaffer; his nephews, Raymond Shaffer (Chelsea) and Joshua Shaffer; and his great-nieces, Raychel and Riley Shaffer.
