He was predeceased by: his parents, Albert H. Cook Sr. and Margaret A. Cook (Martz); and his grandparents, Elmer, Margaret Martz and John.
He is survived by: his son Matthew Cook (Sarah Cook); his sister Shirley Lewis (Marshall Lewis); his grandchildren, Callie, Marli and Grady Cook; his nephew Rob Houser (Jen Houser); his niece Chris Listmann (Brady); and his close friend Joe Rozmarynowski. He is also survived by many cousins, great nieces, great nephews, and dear friends.
In lieu of flowers, the family kindly requests donations be made to St. Jude Children's Research Hospital or the Colon Cancer Foundation in honor of Butch's legacy and his spirit of caring for others.
