He was predeceased by: his parents, Herbert A. and Mary A. Cook (Morris); and his brothers, Herbert and Richard. He is survived by: his wife Donna Cook; his sons, Steven (Stacey) of Sudbury, MA and Thomas of Claymont, DE; and his grandchildren, Christopher and Samantha.
