He is survived by: his wife Mary C. Hall; his children, John IV (Brittany Hall) and Catherine (Sean Fitzpatrick); his grandchildren, John James Hall V, Brooks Michael Fitzpatrick and Beckett Hall Fitzpatrick; his sister Kathleen "Kathy" Carmeans (Wayne); his sister-in-law Teresa Williams (Rick); and his friends, Larry, Matt, Dina and Pat. He is also survived by his niece, nephew, cousins.
In lieu of flowers, donations may be made in Jack's name to Faithful Friends Animal Society or St. John the Beloved Catholic Church.
