He is survived by: his fiancée Angela Plaskett; his children, Nichole Cook, Jacqueline Cook and Michael Cook; his grandchildren, Anthony, Aaron, Sophia, Maddie and Jaxon; and his sisters, Angie, Bonnie and Ginger. He is also survived by many extended family members, dear friends.
