He is survived by: his parents, Christy Polnaszek and Thomas Polnaszek; his sisters, Veronica Polnaszek, Vanessa Smith (Zac) and Breeana Perry; his grandparents, Corky Polnaszek, Ronald Meade and Walter Howard; and also Shelly Meade. He is also survived by several Aunts, Uncles, and Cousins.
