He is survived by: his daughter Heather Dawn Forehand Leonhardt of Wisconsin; his girlfriend Mary Espey of Florida; and his grandchildren, Mason William Scott Forehand of Wisconsin, Savannah Leonhardt of Wisconsin, Phoenix Leonhardt of Wisconsin and Boston Leonhardt of Wisconsin.
