He is survived by: his wife Roberta Mittelberg; his children, Barry Mittelberg (Robyn Magaril), Rickey Mittelberg (Amy) and Leslie Mittelberg (Dale Morse); his grandchildren, Jared Morse (Amy), Tyler Morse (Dlorah), Rachel Cohen (Andrew), Brian Mittelberg (Kristen), Alissa Mittelberg, Ariel Mittelberg and Tyler Mittelberg (Megan); his great grandchildren, Isaac, Rayna, Ryan, Chloe, Joshua, Bradley, Dylan and Connor; and his sister Frances Rabeck.

In lieu of flowers, the family requests donations in Jerome's memory be made to the Jewish Federation or the Anti-Defamation League.
