He was predeceased by: his parents, Charlie Polis and Virginia Polis; and his wife Barbara Polis. He is survived by: his children, Chuck Polis (Debbie), Laurie Smith (Roger) and Sherrie Wirtz (Chris); and his grandchildren, Sarah, Caroline, Chaz, Amber and Aubrey.
