He was predeceased by : his parents, Thomas C. Ballard Sr. and Pauline C. Oswein; his former life partner Patricia Burke; and his brother Donald Ballard. He is survived by : his wife Jan Ballard; his sons, Chris (Marcia), Stephen (Charlie), Michael (Lynn) and Patrick (Elaine); and his cousin Elizabeth Ann Pickerill. He is also survived by 9 grandchildren; 5 great-grandchildren.
