He is survived by: his wife Jewell Cook of Wausau, FL; his son Craig Cook (Lynn) of Wausau, FL; his sisters, Martha Williams of Louisiana and Lunell Hefner (Dave) of Freeport, FL; his grandchildren, Rebecca Cook, Ashley McCall (Nick), Keith Cook (Kasey), John Michael Cook (Nikki) and Madison Cook; and his stepgrandchildren, Gabriel Dizon and Juliana Dizon. He is also survived by 10 great-grandchildren.
