He was predeceased by: his wife Eleanor Yvonne Cook. He is survived by: his children, Gerald Cook (Cindy) of Jacksonville and Karen Cook of Fernandina Beach. He is also survived by extended family including 4 great grandchildren and a great-grandchild.
A funeral service was held on Saturday, December 6th 2025 at 11:30 AM at the New Life Baptist Church (464069 FL-200, Yulee, FL 32097).
