He is survived by: his wife Cynthia Hazen of Fernandina Beach; his children, Leslie Swisher (Jeffrey) of Columbus, GA and Johnathan Hazen of Fernandina Beach, FL; his siblings, Leah Armbruster of Oakland, IL and Robert Hazen of Clarkesville, TN; and his grandchildren, Tyler, Kol and Cayde. He is also survived by several nieces and nephews.
