She was predeceased by: her nephew Jeffrey Forman. She is survived by: her significant other Edward Smith; her children, Todd "TJ" James (Amy) and Stephanie Atkins; her sister Bette Forman; her grandson Christopher James (Renee); her great grandchildren, Charlotte and Jackson; and her nephew Brian. She is also survived by many friends and cousins.
In lieu of flowers, the family has designated Wounded Warriors for memorial contributions.
